Six sigma failed this time

The mailer below arrived in my mailbox Tuesday, May 14, 2019.

It has the 2018 garbage holiday schedule.

How could this happen?

Did the post office deliver them six months late?

Did someone at the city just spend a bunch of money for mailing and printing and not bother to look at what was being sent?

Did the printer make the mistake?

How much did this cost the customers?

Will someone be held accountable?

We deserve better

    Actually, since I got a recycling mailer that was current at roughly the same time, my guess is either someone at the city or the mailing house grabbed a stack of old postcards by mistake. That said an organization using Six Sigma should have a 5S discipline that would drive recycling of out-of-date materials to both prevent this type of error and free up storage space for more productive use. Looks like the comms team now has a green belt project.

    If the city manager is never held accountable and even gets his legal bills paid by us, why should any of the minions have to pay for their mistakes?
